Together with the Geauga County Jail, United Way Services of Geauga County, and Ravenwood Health, and funded by the Cleveland Foundation, Family Pride is striving to support the transition of inmates and their families as they are released from the Geauga County Jail back into the community. This program incorporates a collaborative approach with multiple community partners to address the growing needs of individuals in the jail and, most importantly, reduce persistent recidivism rates of Geauga County residents. Program partners will work together to directly impact both the overall health and emotional wellness of the individuals and their families.
Geauga County Reentry Collaborative
The Geauga County Reentry Program includes onsite case management, life skills, employment and educational support services. Once released participants experience transition services that utilize a strengths-based, individualized approach to meet the needs of each individual and their families by connecting them to needed resources. In addition, all program participants work with a transition care coordinator who will help them meet their goals as well as provide consistent follow-up support and coaching.
