How do I get an appointment with Family Pride?
Either call the Family Pride office at 440-286-1553 or complete the online referral form and Family Pride can contact you directly.
What health insurances do you accept?
Family Pride is in-network for Medicaid insurances (Caresource, United Health Care, Paramount, Buckeye, Molina), Medical Mutual, Aetna, Anthem, and United Health Care. Family Pride accepts and will directly bill to all private health insurances. Clients are responsible for remaining co-pays and deductibles.
Family Pride offers a sliding fee scale for those clients who qualify. If you are a Geauga County resident, you may qualify for a grant support.
How do I make a payment?
All co-pays can be paid by cash or check upon each visit. Clients being seen in the home may send their payments via mail in an envelope provided by their counselor or case manager. Family Pride also accepts all major credit cards which can be taken at the office, over the phone or online. All clients who want to pay out-of-pocket for services will need to pay for services in full prior each session.
Who do you work with?
Family Pride works with children, adolescents, and adults 3 year old and up for outpatient mental health and substance abuse counseling and support. For more information link: "Who Do We Serve".

Family Pride’s office is located at 695 South Street, Suite 6, Chardon, Ohio 44024. Services are provided in the client homes and throughout the community in Geauga, Ashtabula, Lake and portions of Cuyahoga and Portage Counties.
